Short answer is to contact your local intellectual development disability authority. Other wise known as the LIDDA. They will be your first contact to initiate the process. Do remember to bring any supportive documentation regarding the disability. After the initial screening then their is a comprehensive assessment . During this assessment there may several other health care professionals involved to help in the determination of eligibility . Once completed a Person directed plan is created with the individual in mind. A case manager and service coordinator will be assigned. Financial eligibility with Medicaid is determined during the assessment. Depending on the waiver slots available  there may be along wait list , the list is reviewed regularly and the applicant is updated. Once approved your LIDDA will provide list of providers that offer services to care for your needs. If you are interested in getting started please start today do to the wait-list being long.

We have contracted out trained medical staff on-site to provide basic medical services such as medication management, wound care, and monitoring of chronic conditions. For more specialized medical care, we work contract with  local healthcare providers. Which you have the option of selecting for the medical care.

We maintain a high staff-to-resident ratio to ensure that residents receive the attention and care they need. Our staff is available 24/7 to assist with any needs or concerns.
